This experience really gave me a unique glimpse into a role in nursing I had never seen before. The nurses in this role help people with short and long-term goals during their time with hospice as well as helping people with symptom management at end of life.
What I wish was different
I wish i had better opportunities to see those out in the community whom work at Agrace. There is such a wide net of in-home nurse case managers and their role is uniquely different from the case managers at the facilities.
Advice
I would like to share that anyone who does an internship like this needs to ask lots of questions and really push to get yourself involved in all the possible nursing interventions the case mangers do. It is important to be outgoing and really connect with everyone you come in contact with.

What I liked
Great support system from HR and head Nurses. I loved getting to know my clients and getting to help them in the most convenient place for them--their own home, surrounded by friends, family, and their healthcare team.
